What is a medical home?

The “medical home” model of care puts you at the center of managing your health needs. Mercy Clinic physician practices are considered a medical home when they provide services that make the doctor’s office the true hub for all patient care. Your physician leads a team that takes responsibility for helping you manage your health.

Medical home practices provide these benefits:

  • Extended hours for appointments and immediate care needs, including weekend hours may be offered.
  • A team of health care professionals led by your physician and including nurse practitioners, physician assistants and case managers.
  • Dedicated staff to evaluate your needs, schedule appointments, arrange referrals to specialists, coordinate care for long-term treatment needs and provide patient education.
  • Electronic access through email to schedule appointments or obtain information regarding your care.
