  • Disease management specialists monitor how you are doing with your health, and what might need to be changed to keep you well.
  • Case managers focus on your individual needs and ensure you are receiving preventive care services between doctor visits.
  • Nurse-on-call services help you decide the most appropriate care.
  • E-visits allow you to visit with your doctor using an online consult.

Innovation in Action

You exchange information online with your doctor about your diabetes. You receive your test results on your phone with MyMercy. Home-based technology helps you monitor blood pressure, EKGs, blood glucose and medication compliance. All results flow directly into your electronic health record; they are available to anyone on your Mercy care team involved in your ongoing care.
