Catherine McAuley Physician Award Nomination Form 2020

Due to the COVID19 crisis the physician's award dinner is currently postponed until the fall of 2021, but we certainly welcome your continued nominations. 

Patients, families and visitors, physicians, all co-workers and volunteers, of Mercy Hospital Jefferson are invited to nominate a deserving physician for the Catherine McAuley Physician Award.

This award will annually recognize a practicing physician who demonstrates significant engagement in the Jefferson community in the spirit of Mercy’s Mission and Values:

As the Sisters of Mercy before us, we bring to life the healing ministry of Jesus through our compassionate care and exceptional service.

Dignity: We cherish each person as created in the image of God
Justice: We pledge to be in right relationship with one another with particular concern
             for people who are economically poor
Service: We seek out and put the needs of others first
Excellence: We give only the best for those entrusted to our care
Stewardship: We wisely use our talents and resources to strengthen Mercy as a 
                      ministry of the Church
 
In addition, the honoree must have served on staff of Mercy Jefferson for at least three years.

The award winner will be announced at the Catherine McAuley Physician Award Dinner Fall of 2021.
