Release of Information Form (Medical Records)
Cooper Clinic physicians who have joined Mercy will continue to have access to your medical records. To transfer your records to a non-Mercy physician, you will need to complete a Release of Information form. That form may be submitted at any Mercy Clinic, including those where former Cooper Clinic doctors now practice.
