GME Transitional Year Rotations

Internal Medicine Ward

Each TYP resident spends 2 months on inpatient Internal Medicine.  Of the 2 months, 2 weeks are spent on night float doing admissions to the hospitalists teaching service with supervision provided by in house hospitalists.  The remaining 6 weeks are spent on the daytime hospitalist teaching service working with a team consisting of a teaching attending, an upper level IM or FM resident and another PGY-1.  Each PGY-1 carries 6-8 patients and rounds daily with the teaching attending. PGY-1’s stay until 8 PM as short call every 3^rd to 4^th night. Residents attend IM daily noon conferences and weekly IM Grand Rounds during this rotation (except while on night float).

Ambulatory Medicine

Each TYP resident spends one month on the Ambulatory Medicine rotation. Five ½ days are spent seeing Internal Medicine patients in the JFK clinic under the supervision of the IM clinic teaching faculty.  Five ½ days are spent seeing Pediatric patients in the JFK clinic under the supervision of the Pediatric clinic teaching faculty.  Each resident is required to give a PowerPoint presentation on an Internal Medicine ambulatory topic at the end of the month.  Residents attend IM daily noon conferences and weekly IM Grand Rounds while on this rotation.

Trauma Neuro ICU

Each TYP spends 1 month on the Trauma Neuro ICU (474) on a team consisting of a Neurointensivist teaching attending, CCM fellow, and sometimes a Surgical Intensivist/Trauma Surgeon. The TYP carries 3-5 patients and rounds daily with the team.  PGY-1’s do not take call, but will stay until 8 PM every 4^th night as part of a short call system.

Emergency Medicine

Each TYP does a 1 month EM rotation during which he/she works 24-25 ten hour shifts (rotating between all three shifts).  The TYP sees patients in both the main ED and in Area 2 (where minor emergencies are treated) under the supervision of the EM teaching faculty.  A series of EM didactic lectures and hands on workshops are provided by the attending staff. Residents also participate in a simulation educational experience during this rotation.

Surgery/Trauma

This is mainly an operative surgical experience.  Each TYP scrubs in on General Surgery cases the first two weeks.  The second ½ of the month, the TYP scrubs in on a variety of surgical subspecialty cases pertinent to his/her interests and his/her categorical residency.  Residents attend JFK Surgery Clinic and also Hyperbaric and Wound Clinic during the month.  TYP’s take overnight Trauma call about once a week and do not come in until 5 PM on call nights. Residents attend lectures on Wednesdays and Fridays.  Each TYP is also required to give a PowerPoint presentation of a surgical topic during the month.

Psychiatry

During the first three weeks of this rotation the TYP attends didactic sessions with the Psychiatry Teaching attendings in addition to selected patient encounters. The last week of the month is spent doing Psychiatric consults with the Psychiatry Hospitalist. Each TYP gives a PowerPoint presentation on a psychiatric topic of his/her choice at the end of the month.

Coronary Care Unit

Each CCU teaching service consists of a Cardiology teaching attending, an upper level IM resident and 2 PGY-1’s.  In addition to daily teaching rounds with the Cardiology teaching attending, there are also didactic sessions on EKG interpretation and selected Cardiology topics given 4-5 days per week.  Each resident is required to pass an EKG exam at the conclusion of the month.  PGY-1’s stay until 8 PM every 3^rd to 4^th night as part of a short call system.

Med/Surg ICU

Each ICU teaching service consists of a CCM teaching attending, a CCM fellow and one PGY-1.  Each PGY-1 carries 5-6 patients and participates in daily teaching rounds.  Residents attend daily CCM noon conferences and weekly CCM Grand Rounds. Residents do not take call on the ICU rotation.

Electives

Each TYP has 3 elective months, and TYP’s are strongly encouraged to take electives that will enhance their knowledge base for their chosen categorical residency. In addition, research electives are encouraged.

• Radiology - recommended electives include Neurology/Neurosurgery, Vascular Surgery, GI/Colorectal Surgery, Breast Surgery, Office or inpatient orthopedics, Pulmonology.
• Dermatology - recommended electives include Dermatopathology, Dermatology, Rheumatology, Burn Unit.
• Ophthalmology - recommended electives include Cardiology, Neurology, ENT, Ophthalmology
• Radiation Oncology - recommended electives include Oncology, Radiation Oncology
