Patients must complete our form entitled “Authorization for Release of Protected Health Information” for all release requests and mail or deliver it to the address below. If you would like your medical records sent directly to a physician’s office, in order for you to receive care from that physician, there is no charge for your records to be released. If you request your medical records for your personal use, or to request they be sent to a third party that is not a physician’s office (i.e. an attorney), Mercy Hospital St. Louis (formerly St. John's Mercy Medical Center) & Mercy Hospital Washington (formerly St. John's Mercy Hospital) may charge you a fee for each page of your record that is released to you.
Please note: Mercy Hospital St. Louis & Mercy Hospital Washington does not accept faxed or e-mailed authorization forms. Please mail or deliver completed authorization forms for both Mercy Hospital St. Louis and Mercy Hospital Washington to:

Mercy Hospital St. Louis & Mercy Hospital Washington charge a fee for each page of your medical record according to a fee schedule that is set by the State of Missouri Department of Health and Human Services. A retrieval fee may also be charged for any non-patient requesters (i.e. attorneys).
We accept payment in the form of cash, check or credit card (MasterCard, Visa, Discover, American Express).
