Weight Loss Surgery Criteria

  1. Current BMI of 40 or greater or BMI of 35-39 with obesity-related comorbidities.
  2. 3-5 year history of obesity despite failed weight loss regimens.
  3. Patient who can demonstrate a lifetime commitment to a surgical weight loss program with nutritional guidelines and follow‐up appointments.
  4. Letter of referral from primary care physician with as much health information as possible.
  5. Verification from insurance provider that weight loss surgery is a covered benefit or the patient is willing to self finance.
  6. No drug or alcohol addiction and no poorly controlled mental disorder that would interfere with appropriate follow‐up regimens and compliance.
  7. Must quit smoking for at least two months prior to surgery.
  8. Minimum age of 18.
