  • You require assistance with pain control
  • You have unrelieved symptoms such as pain, shortness of breath, diarrhea, constipation, nausea or vomiting
  • You have a serious illness regardless of your life expectancy
  • You refuse medications due to their side effects
  • You have frequent hospitalizations
  • You have advanced dementia
  • You require total parenteral nutrition, or negative pressure wound therapy
