Pre-registration 

Please print out our pre-registration form and fill it outcompletely before your appointment. This will help expedite the registration process. Please bring the forms with you to your first appointment, along with a photo ID, your current insurance card(s) and all medications that you are currently taking on a routine basis. This includes: prescription medications, vitamins, supplements, aspirin, fish oil, etc. We also recommend bringing the bottles with you – not just a list.
