As a participant, you will have scheduled appointments with your physician, a registered dietitian, and a registered nurse. You’ll learn to manage your diabetes through proper nutrition; exercise; stress management and coping skills; medical care; and the motivation you need to change your life for the better.
We offer small group classes and individual sessions for diabetes self management education for:
- Type 1 diabetes
- Type 2 diabetes
- Gestational diabetes
We work to help you successfully manage your diabetes through:
- Meter training
- Insulin training
- Understanding diabetes medications
- Blood glucose targets and review of lab results
- Exercise guidelines
- Causes and treatments for hyperglycemia and hypoglycemia
- Meal planning
- Prevention of chronic complications
- Support groups
Weight Management and Nutrition
One on one nutrition counseling is provided by our registered dietitian which includes an individual nutrition assessment with meal planning and goal setting. Sessions can provide medical nutrition therapy for the following:
- Weight management
- High cholesterol
- Cancer
- High blood pressure
- Gestational diabetes
- Kidney disease
- Type 1 and Type 2 Diabetes
- Celiac disease, gluten sensitivity and other food allergies
- Gastrointestinal disorders
- Malnutrition and nutrient deficiencies
