How the Program Works
This program allows you to participate from anywhere using your smartphone or computer.
You’ll begin with frequent support to get you going, then less frequent touchpoints as you continue toward your weight loss goal.
You’ll have weekly visits for 12 weeks, then every other week for three months, and monthly check-ins for up to two years. You can join extra visits anytime for a refresher.

It’s a virtual program that helps you build healthy habits for weight loss and overall well-being. The program is fully connected to your Mercy primary care team, so your progress is part of your medical record. You’ll start with 12 weekly group visits led by a Mercy coach, then continue with ongoing support through MyMercy.
The program is designed for patients who have diabetes, a BMI of 30 or higher and are enrolled in a qualified health coverage program.

Each week builds on providing step-by-step lifestyle habits that support weight loss for life.
You’ll learn how to:

- Maintain your progress for the long term
It’s part of your Mercy care, so your progress is tracked in your electronic medical record and your care team is kept in the loop. You get personalized coaching and flexible virtual visits for long-term success.
You'll start with group visits once a week for 45 minutes over 12 weeks. After that, there will be two visits every other week, followed by shorter, 15–20-minute check-ins every two to four weeks using automated messages and helpful tools to support your progress.
There’s no additional cost for the program. Items such as a home weight scale or blood pressure monitor are optional for you to purchase.
You’ll log weight and habits weekly in MyMercy. Blood pressure is entered at your first visit and again at 12 and 24 months.

Your blood pressure consists of two numbers, written like this: 120/80 or 120 over 80. The top larger number is the systolic pressure. It measures the pressure in the arteries when the heart beats or contracts. The bottom smaller number is the diastolic pressure. It measures the pressure in the arteries between heartbeats – when the heart muscle is resting and refilling with blood. Learn how to accurately measure blood pressure.
This program will require you to take blood pressure readings three times. It’s best to use a device that measures your blood pressure on your upper arm. We recommend purchasing one from the list of validated monitors here. You may also find options to get your blood pressure taken in your community. Here are some options: mercy.net/BPcheck.
