Your primary care teams at Mercy represent a partnership between your physician and many talented and dedicated team members at Mercy.  These individuals join their talents and experience to assist you in meeting your personal goals for your health. Your Mercy Primary Care Team is here to answer your questions, coordinate your care, and assist you in making choices that impact your health 24 hours a day, 7 days a week.  Look below to see the roles and responsibilities in Primary Care.

Primary Care Team Roles

Primary Care Physician

Your physician is the coach or leader of the care team.  They counsel you on the options and work with other physicians and care members to ensure you’re getting the right care at the right time.  Your physician has extensive Medical education and experience.

Nurse Practitioner, Physician Assistant

A Nurse Practitioner or Physician Assistant is a member of the team that extends the services of the physician.  In many instances, these providers will see you in the office, order your tests and counsel you on the best care services for you.  These care team members work closely with the physician in reviewing your care needs routinely.

Case Manager

Case managers play an important role in helping our sickest patients between office visits.  These individuals are trained nurses with talent in understanding and caring for patients needs while you’re at home.  They can offer medical advice after hours, coordinate services on your behalf, and coach you to achieve your best health.

Clinic Nurse

Your clinic nurse can be a Registered Nurse (RN), Licensed Practical Nurse (LPN) or Medical Assistant (MA).  Your clinic nurse is the member of the care team who works hand in hand with the physician or providers to meet your care needs during your visit.  Nurses connect all the dots between providers, patients and care managers on your behalf.  They are often the go-to resource for information in the clinic. 

Patient Service Representative

A patient service representative is your first point of contact when reaching the clinic.  These individuals ensure your appointments meet your needs.  They will work with you and the provider to ensure you’re seen by the right person at the right time to meet your needs.  These people also play a vital role in keeping the practice organized and running smoothly so we can provide you the very best Mercy Experience.
